**Note: Please fill out the following form completely. Print out two copies, one for your personal records and the second to be sent to us with your order. Once we have received your order, we will verify the quantities, options specified and the final price.

		**DVD Slide Shows should be limited to no more that 450 photo's. Please sort into chapters and provide titles. Also, provide a legal copy of the music required in a digital format. Music can be provide for you for an additional fee.
